Connections at Landow House offers the optimal balance of vibrant independence with dignified support. Personalized care is delivered in the active, engaging environment of assisted living, making Connections at Landow House an ideal residence. It’s just the right amount of support in just the right setting to help residents thrive with a sense of independence, purpose, and fulfillment.

Our nationally recognized memory care residence at Cohen-Rosen House combines an innovative setting with a family-style approach to help residents maintain a meaningful and enjoyable quality of life.

Residents of Cohen-Rosen House enjoy a safe, serene, and caring home specifically designed to meet the needs of those living with Alzheimer’s disease or other dementia-related disorders. Residents experience the benefits of a unique setting where private accommodations, creative programming, and personal solutions shape each day. The experience is designed to maintain important connections in life – family, faith and values, caregivers and neighbors, the natural environment, comfort, and security. Our professional team members are trained and skilled in dementia best practices, and their responsibilities are performed to support a traditional household.

Receive specialized medical care with our outpatient care.

Older adults require convenient access to exceptional medical care and rehabilitative services uniquely focused on their needs to remain healthy and independent. That’s why we opened the Harold & Jane Hirsh Health Center – a medical practice that specializes in primary geriatric care. Both residents of the Charles E. Smith Life Communities and older adults from the surrounding area can receive the highest-quality care possible delivered by primary care. Our outpatient services provide 24-hour physician coverage through an on-call service after hours, appointments with visiting specialists and so much more.

Return to health and return home with our rehabilitation services.

Following an illness, accident, or surgery, older adults may require a little extra support to return home and live independently. At the Warren R. Slavin Post-Acute Care Center at Hebrew Home of Greater Washington, we focus on the lifestyle of each individual and make their goals our priority. With comfortable, private accommodations and a team of over 30 experienced therapists, it is no wonder that so many of our short-stay patients achieve one of their greatest goals – returning home. We have over 30 licensed therapists with certifications from national associations in six specialty areas: specialized exercise, orthopedics, geriatrics, stroke interventions, cognition, and Parkinson’s disease.
